Your rights and responsibilities

This is a statement of your rights and responsibilities as a patient at this practice; we are keen to work with you in providing the best care possible.

Your rights are:

  • To be registered with a family practice
  • To be offered a health check when joining our practice list for the first time
  • To receive emergency medical care or advice at any time through the family doctor service
  • To have all the medicines on prescription which your doctor feels are necessary and appropriate
  • To be referred to a consultant when your family doctor thinks it is necessary and to be referred for a second opinion if you and your family doctor agree that is desirable
  • To have access to your own health record, subject to any limitations in law
  • To know that those working for the NHS are under a legal duty to keep the contents of your health records confidential
  • To choose whether or not to take part in medical research or medical student training
  • To receive a copy of the practice leaflet, setting out the services provided
  • To have access to a regularly updated and interactive practice web-site
  • To receive a full and prompt reply to any complaint you make about NHS services
  • To change your practice quickly and easily where the doctor/patient relationship has broken down irretrievably
