New Patient Registration
For new patients there is a defined registration process. We will need to ensure that you are within our practice boundary. .
Once confirmed, download the registration form and Health Questionnaire from the above "Registration" tab (you will need one copy for each family member) or ask for copies at reception. You can complete these electronically but they do need to be signed so will have to be printed off and brought to the surgery.
When completed for all family members that you wish to register come to either Tile Cross or Church Road Surgery reception in the afternoon, during surgery opening times . For each family member, if possible, bring photo ID and evidence of the family address (bank statement, utility bill, etc.) with your completed Registration forms.
All new patients will require a New Patient Health Check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Additionally Patients will be requested to register for On-line prescription and appointment access.
Medical treatment is available from the date of registration. Please contact reception for further information.
